PI 19 - Kidney function test result

Proportion of First Nations regular clients with a selected chronic disease who have had a kidney function test result (eGFR and ACR) within a specified category.

Description

Proportion of regular clients who are Indigenous aged 18 years and over, who are recorded as having one of the following:

  • Type 2 diabetes
  • Cardiovascular disease (CVD)
  • Type 2 diabetes and/or CVD
whose estimated glomerular filtration rate (eGFR) AND albumin/creatinine ratio (ACR) recorded within the previous 12 months were categorised as:
  • Normal risk
  • Low risk
  • Moderate risk
  • High risk
Table 1. eGFR & ACR risk categories
Risk level Description
Normal risk eGFR ≥60 mL/min/1.73m² AND ACR <3.0 mg/mmol
Low risk Either:
  • eGFR ≥45 mL/min/1.73m² and <60 mL/min/1.73m² AND ACR <3.0 mg/mmol, OR
  • eGFR ≥60 mL/min/1.73m² AND ACR ≥3.0 mg/mmol and ≤30 mg/mmol
Moderate risk Either:
  • eGFR ≥45 mL/min/1.73m² and <60 mL/min/1.73m² AND ACR ≥3.0 mg/mmol and ≤30 mg/mmol, OR
  • eGFR ≥30 mL/min/1.73m² and <45 mL/min/1.73m2 AND ACR ≤30 mg/mmol
High risk Either:
  • eGFR ≥30 mL/min/1.73m² AND ACR >30ml/mmol, OR
  • eGFR <30 mL/min/1.73m² AND any ACR result

Notes

  • User may select between AIHW's definition of Regular Client (attended the OATSIH-funded primary health care service at least 3 times in 2 years), or Communicare's Current Patient status.
  • Patients must be recorded as Aboriginal or Torres Strait Islander or both.

Numerator

The number of [Disaggregation: Sex] First Nations regular clients with [Disaggregation: Chronic disease] aged [Disaggregation: Age] whose most recent eGFR and ACR results, recorded within the previous 12 months, were [Disaggregation: Risk category]

Note:
This is equivalent to calculation A to L on METEOR
Denominator Total number of [Disaggregation: Sex] First Nations regular clients with [Disaggregation: Chronic disease] aged [Disaggregation: Age] who had both eGFR and ACR results recorded within the previous 12 months
Disaggregation
  • Age
    • 18 to less than 25 years
    • 25 to less than 35 years
    • 35 to less than 45 years
    • 45 to less than 55 years
    • 55 to less than 65 years
    • 65 years and older
  • Sex
    • Male
    • Female
  • Chronic disease
    • Type 2 diabetes
    • Cardiovascular disease
    • Either or both of the above
  • Risk, see Table 1.
Counting rules Inclusion Criteria
  • Have type 2 diabetes and/or CVD
  • A valid eGFR AND a valid ACR test recorded within the previous 12 months
Exclusion Criteria
  • Type 1 diabetes, secondary diabetes, gestational diabetes mellitus (GDM),previous GDM, impaired fasting glucose, and impaired glucose tolerance
  • Invalid test results (i.e. negative, non-numerical and null values)
Additional data recording considerations
  • Count is of people, not tests
  • Count only the most recent eGFR and ACR tests
  • 'ACR' definition: albumin/creatinine ratio
  • Valid test result recorded: a numeric value that can be used to classify test results into the risk categories specified in ‘Chronic Kidney Disease (CKD) Management in Primary Care’ guidelines
  • Chronic disease patient counts:
    • Type 2 diabetes: have type 2 diabetes irrespective of CVD status
    • CVD: have CVD irrespective of type 2 diabetes status
    • Type 2 diabetes and/or CVD: have diabetes only OR have CVD only OR have type 2 diabetes AND CVD
